Inject depth guidance into the synthesis prompt based on tier:

small: concise, reference actual filenames, 2–3 paragraphs
medium: structured breakdown, cover purpose/components/concerns
large: thorough architectural analysis with section headers
xlarge: comprehensive report — architecture, subsystems, interfaces, cross-cutting concerns, notable anomalies
