For large tier: add a grouping pass between dir loops and final synthesis.

Grouping agent receives all dir summaries and identifies 3–10 logical subsystems (not based on directory structure — based on function and relationships). Each subsystem gets a summary.

Final synthesis receives subsystem summaries instead of hundreds of dir summaries.
